Bramacare shares its risk assessment process for World Patient Safety Day

Ruben Russell, Deputy Manager at Bramacare, has shared some of the ways service users are kept safe throughout their recovery journey at The White House and Bramacare Community.

In a short video produced for World Patient Safety Day (17 September), Ruben explains the risk assessment process at Bramacare and how risks are reviewed frequently to meet the individual and ever-changing needs of service users.

Ruben explains: “First, we liaise with the referral team to understand their needs and what the service user would like to achieve from their placement at The White House. After we liaise with their home team, we assess the person’s suitability for joining Bramacare. We prefer to assess in-person so they can meet our team and see what we offer here.

“After we have all the information we need from the referral team and the service user, we create a care plan and risk assessment to meet their individual needs. When the service user is admitted, we have a screening period of a few weeks to ensure they are adapting and settling in well, and that their care plan and risk assessment is appropriate. We then continue to review the risk assessment regularly to ensure everyone’s safety.

“Our team at Bramacare are all trained with the skills and experience needed for risk assessments. We also involve service users in their own risk assessment to promote accountability and responsibility throughout their rehabilitation process.

“We use a digital cloud-based system, Access Care Compliance, to securely store all our risk assessments and care plans. Digital records for all our patients minimise the risk of error and ensure all records can be tracked and updated effectively and securely. 

All service users are different, they have different goals and needs, and it’s our mission to personalise their care plans as much as possible to enable them to achieve what they’d like to achieve. Their needs keep changing so our risk assessments and safety measures and protocols also change.”
